Buffalo Trace Area Development District is seeking applicants to serve in a case management role for elderly and disabled individuals.
Responsibilities include, but are not limited to:
- Completing monthly home visits, maintaining client and service documentation, client engagement and communication, planning and goal setting, monitoring and coordination.
- Case loads will be contained in a limited area within the five county district comprised of Bracken, Fleming, Lewis, Mason and Robertson counties.
- Case Managers will work with clients and or their representatives to assure compliance with program regulations, policies and procedures and be able to provide technical assistance.
Qualifications:
- The preferred candidate must have: Bachelor’s Degree in a human services field from an accredited college or university or bachelor’s degree in any other field from an accredited college or university with one year’s experience in the field of intellectual disability or Registered Nurse currently licensed as defined in KRS 314.011(5) who has one (1) year or more experience as a professional nurse in the field of intellectual disabilities.
- Required skills include: excellent verbal and written communication, self direction, strong organizational skills, efficiency, experience and knowledge of Microsoft office programs, and possess an ability to relate to consumers and other professionals.
- Applicant must possess a valid drivers license.
